A 25yr old man came with painful skin lesions over the soles and fingertips since 4 days and burning sensation over epi gastric regions since 1 month

Chief complaints

The patient came with painful lesions over the soles and fingertips 

History of Presenting illness

Patient was apparently alright 4days back came with painful skin lesions over the sole and fingertips 

-Burning sensation over the epi gastric region associated with cough and headache since 1 month

-Abdominal tightness present

History of Past illness

-No similar complaints in the past

PHYSICAL EXAMINATION

A. GENERAL

Pallor- no

Cyanosis - No

Icterus - No

Clubbing of fingers/toes - No

Lymphadenopathy - No 

 Malnutrition - No 

Oedema of feet - No

Dehydration- No.

Pulse rate-95bpm

Bp: Lf arm: 80/100mmHg

3. Cardiac murmurs No 

C. RESPIRATORY SYSTEM:

2. Wheeze - No

4. Breath Sounds - Vesicular

D. ABDOMEN

1. Shape of abdomen - Scaphoid

2. Tenderness - No

3. Palpable mass - No 

4. Hernial Orifices - Normal 

5. Free Fluid - No 

6. Bruits No 

7. Liver - Not palpable 

8. Spleen - Not palpable 

9. Bowel sounds - No

E. CENTRAL NERVOUS SYSTEM

A 48yr old male with vomplaints of vomitings

Chief complaints:- The patient came to the general medicine op with chief complaints vomitings since 4 days History of present illness:- The patient was apparently asymptomatic 1 month back.Then he developed neck,back(pelvic),lower limbs pain of dragging type for which he came to kims hospital.The pain got aggrevated on work and got relived on rest. RFT was done and patient was advised to undergo  dialysis. But the patient then ignored and did not undergo dialysis. 4 days back the patient had few episodes of vomiting after eating,which is non projectile,non bilious and didn’t contain any blood,he then visited Kims hospital where 2 rounds of dialysis was done.  History of past illness:- The patient is  a known case of

 Hi,I’m Rohith.N 5th semester medical student.This is an online e block to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio A 35 year old female came to OP with fever and weakness  Chief complaints: The patient came to the general medicine op with chief complaints of fever, weakness, shortness of breath since 4days  History of present illness The patient was apparently asymptomatic 3 months back.Then she developed fever associated with chills for which she went to a local hospital where she was diagnosed with severe anemia(3gm/dl) then she used medication for 1 month. Then 1 month back the patient developed fever and weakness and shortness of breath which got aggravated on exertion and relieved on rest.4 days back she Kims hospital and she was administered 2units of blood and 1unit of iron.The haemoglobin level rise up to 8gm/SL History of past illness: The patient is not a known case of diabetes,asthma,epilepsy
