A 48yr old male with vomplaints of vomitings

  Hi,I am N.Rohith (roll no 79) 5th semester medical student.This is an online e block to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio A 48 year old male came to OP with complaints of vomitings. Chief complaints:- The patient came to the general medicine op with chief complaints vomitings since 4 days History of present illness:- The patient was apparently asymptomatic 1 month back.Then he developed neck,back(pelvic),lower limbs pain of dragging type for which he came to kims hospital.The pain got aggrevated on work and got relived on rest. RFT was done and patient was advised to undergo  dialysis. But the patient then ignored and did not undergo dialysis. 4 days back the patient had few episodes of vomiting after eating,which is non projectile,non bilious and didn’t contain any blood,he then visited Kims hospital where 2 rounds of dialysis was done.  History of past illness:- The patient is  a known case of

A 35yr old female came with complaints of fever

 Hi,I’m Rohith.N 5th semester medical student.This is an online e block to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio A 35 year old female came to OP with fever and weakness  Chief complaints: The patient came to the general medicine op with chief complaints of fever, weakness, shortness of breath since 4days  History of present illness The patient was apparently asymptomatic 3 months back.Then she developed fever associated with chills for which she went to a local hospital where she was diagnosed with severe anemia(3gm/dl) then she used medication for 1 month. Then 1 month back the patient developed fever and weakness and shortness of breath which got aggravated on exertion and relieved on rest.4 days back she Kims hospital and she was administered 2units of blood and 1unit of iron.The haemoglobin level rise up to 8gm/SL History of past illness: The patient is not a known case of diabetes,asthma,epilepsy

A 73yr old male with tremors

A 73yr old male came to the hospital with complaints of tremors Chief complaints: The patient came to the hospital on tuesday with chief complaints of shivering of hands and legs History of presenting illness: The patient was apparently asymptomatic 1yr ago then mild tremors of upper limbs has begun,which then eventually tremors of lower limbs has also begun.Initially the severity of the tremors are mild later the intensity has increased.There has been tremors of upperlimb even while eating.The severity of the tremors increased when the patient was holding any object and decreased while sitting idle. Negative history: Fever- No history of fever Shortness of breath: No history of shortness of breath  Past history: No significant past history  Personal history: Married Occupat

A 50yr old female came to hospital with high grade fever and swelling of legs and hands 3days ago Chief Complaints The patient came to the hospital on Saturday with chief complaints of high grade fever since 4days ,swelling of feet and hands since 3days  History of present illness The patient went to suryapet government hospital before where she was told she had dengue,malaria,low blood volume.On Saturday she came to kims,tests were done and was told there was only low platelet count.One day back a kidney scan was done and found that there was a kidney problem.The patient is not aware of the kidney problem.
